I have been following this discussion related to terminology for many years. From 1919 to 1957 the term cost accountant was popular as indicated by the titles N.A.C.A. Bulletin and N.A.C.A Official Publications. (http://maaw.info/NACABulletin.htm)
In 1957 the term cost accountant was viewed as too narrow and the N.A.C.A. became the National Association of Accountants and its publication became the N.A.A. Bulletin. (http://maaw.info/NAABulletin.htm)
There were various attempts to define management accounting in the 1960s For example, see Singer, F. A. 1961. Management accounting. The Accounting Review (January): 112-118. ([http://links.jstor.org/sici?sici=0001-4826%28196101%2936%3A1%3C112%3AMA%3E2.0.CO%3B2-S)], and the terms management accounting and management accountant were viewed as more appropriate. In 1965 the NAA changed the title of its publication to Management Accounting. (http://maaw.info/ManagementAccounting.htm)
For a while it appeared that the term controller and controllership would catch on (See Controllership & Treasurership Functions for the Financial Executives Institute's 1962 distinction made between accounting and finance - http://maaw.info/ControllershipTreasurership.htm), but the term management accounting remained popular.
The following articles (and many others) continued to promote the term management accounting. Keller, I. W. 1976. All accounting is management accounting. Management Accounting (November): 13-15. National Association of Accountants. 1981. MAP committee promulgates definition of management accounting. Management Accounting (January): 58-59. In the 1990s the term "accounting" had acquired an unfavorable connotation and many practicing accountants no longer referred to themselves as accountants or management accountants. Recognizing this trend (from survey and other results), the IMA changed the title of its publication to Strategic Finance. (http://maaw.info/Strategic%20Finance.htm)

A recent IMA definition seems to indicate that the terms "management accounting" and "management accountant" have again become the popular terms.
